• New York State tax form instructions

For the registration form, you can click on the first field (company name) and tab through the rest of the form. The boxes where you signify what you vend are check boxes and can be set with a click or with the spacebar. Make sure you also correctly set the number of tables requested. If you have any questions, you can email dealers@iconsf.org and we will try to answer them.

The form must be mailed with your check to the address listed. Unfortunately, we do not yet have on-line reservations.

Out of State Vendors:

Do not mail the sales tax form to Icon. That form is included for your convenience only. You must deal directly with New York State for sales tax registration.
